Care Coordination Network

SWCAP Care Coordination Network focuses on connecting individuals to local medical, behavioral health and social service programs in an effort to improve the health and well-being of individuals and families in our community by addressing social determinants of health barriers (SDOH). SDOH are the conditions in the environment where people live, work and play that affect health, functioning and quality-of-life and contribute to health and social inequities. Examples of SDOH include:

  • Health care – medical, dental and behavioral health services
  • Housing
  • Employment and Income
  • Education
  • Food Security
  • Safety – such as safe housing and neighborhoods
  • Discrimination
  • Language and Literacy

The SWCAP Care Coordination Network consists of Community Health Workers who meet with individuals one-on-one to assess their specific needs and connect them to resources such as medical and dental providers, behavioral health providers and other social service programs such as food pantries, housing and transportation to address SDOH barriers. Not only do Community Health Workers connect individuals to services, they follow up to make sure needs are met on a continuous basis. Here’s a list of just some of the things that Community Health Workers can assist with:

  • Finding medical, dental or behavioral health providers and helping to schedule appointments
  • Arranging transportation to and from appointments or for other things like grocery shopping or attending church
  • Addressing food security and access to food pantries and the Women-Infant-Child Food Program
  • Assisting with housing resources
  • Coordinating services for weatherizing your home
  • Translation services
